Attached is my profile for this fixture from Amazon:

The profile name matches what the fixture has on its nameplate even though the manual isn't named the same.

(Edit 2019-10-07: Added default of 127 to pan/tilt channels.)
(Edit #2 2019-10-07: removed default values again - they were messing with the Simple Desk.)
