Fixture Request - Lixada PAR12 LED 15W

I am new to all this, so figured I would ask for some help. Got a batch of LED PARs on eBay to play around with. The box shows Lixada and PAR12 LED. I believe they are the ones at this link https://www.lixada.com/p-l0374.html.

I couldn't find a manual online, but a took pictures of the paper one and made that attached PDF.

I think I might have figured out what I was doing wrong. Will post the file when I get it all working.
